When the health and welfare of your friend or family member is involved, it's important to have a plan that coordinates all quality of life services. Family Private Care works closely with the geriatric care managers and social workers in our service area. These professionals work closely with healthcare providers, elder law attorneys, financial practitioners, and a carefully screened network of community resources to provide the information you or your family needs to make informed decisions - even if you live several states away! They will assess the person's mental, physical, environmental, and financial condition, then prepare an objective, written plan of care tailored to the specific needs of the patient. They will also coordinate the services of all the appropriate providers.
